Changing and Improving EO Example — 250-Word Statement
At EO level, assessors expect more than openness: they want you to have suggested a practical improvement and helped implement it. Show a small piece of analysis (what was wrong, how you knew), your personal actions during the change, and a measured result. Feedback you acted on should appear naturally in the story.
Example statement
SI was a senior care assistant in a 40-bed residential home. Daily notes were written in one shared diary, which made it hard to find information quickly during handovers between shifts.
TAs the most experienced assistant on my shift, I felt responsible for suggesting a safer and clearer way of recording care.
AI raised the issue at a team meeting and suggested moving to individual resident daily sheets. The manager asked me to draft a template, so I looked at what information the nurses asked for most and built the sheet around those items. I piloted it with five residents for a fortnight and asked colleagues each day what was missing or unclear. Based on their feedback, I added a tick box for fluid intake and moved the mobility section to the top of the page. One colleague found the change difficult, so I paired with her on two shifts and we completed the sheets together until she felt confident. I also compared our draft against the recording requirements in our care plans to make sure nothing essential was lost. I checked with the night staff as well, because they used the notes differently, and added a short section for overnight observations. When the trial ended, I wrote a short summary of what worked and presented it to the manager.
RThe home adopted the sheets for all 40 residents, handover time fell by around ten minutes per shift, and our next inspection rated our record-keeping as good.

Common mistakes at this level
- Suggesting an idea but showing no role in making it happen
- Skipping the evidence: no sign of how you knew the process was weak
- Writing 'we decided' where the reader needs 'I proposed'
Frequently asked questions
At AO, openness and ideas are enough. At EO, assessors want you to have suggested a specific improvement and helped implement it — designing a fix, testing it, or supporting colleagues through the change. If your example ends at 'I suggested it and my manager did the rest', it reads as AO level.
A team process you used daily and improved: a check-in flow, a record-keeping method, a rota, a booking routine. Show how you knew it was weak, what you proposed, what you personally did to trial it, how you adjusted from feedback, and what the numbers said afterwards.
Build one genuine feedback moment into the story. Perhaps a colleague flagged a flaw in your idea and you changed the design, or your manager suggested an addition you welcomed. Name what changed because of the feedback — the adjustment is the evidence, not the claim that you welcome it.
